Thank you for your interest in registering your youth for the summer program at our Fortville Extension.

At this time, our site is experiencing issues in registering new members to the Fortville Extension, so we ask that you download/print/complete the forms linked to the button below and email to admin@bgchc.com or drop off, starting April 1st, to our Jim Andrews Unit site at 715 E. Lincoln St. Greenfield, IN 46140. You may also pick-up forms from this site if unable to print from home.

We apologize for the inconvenience until our site is back open to register. Registrations will be entered into the system in the order they were received. Our maximum capacity each week will be limited to 40 registrations. Registrations received after hitting that number will be put into a waiting list order.

Forms must be completed for each registered member (pages 1-3). Additional pages can be copied to reduce time.

Please include in your email the Weeks you expect your youth to attend.
